Navigating the medical system can not only be challenging and complex to the individual, but also for their families and/or support system. I hope to share my years of experience to help people navigate advance care planning and tailor a plan specific to them. By having conversations and completing important documents before a life altering event you can relieve a burden off your loved ones while ensuring your wants and wishes are carried out.
Grief is another challenging and complex experience to navigate. Grief is not about getting over a loss or forgetting about it. Instead it can be about creating a safe and therapeutic space to process, adapt and honor loss.

Advance Care Planning Consultation
Planning for the future can be scary and overwhelming. I can help facilitate those discussions and capture your wishes in appropriate documents. While documents like advance directives are available to the public they can be overwhelming to complete.
Services:

Facilitation of discussion to explore goals and preferences related to aging, illness, death and dying

Education and assistance in completing advance care planning documents

Support and assistance having advance care planning discussions with your family or for family wishing to discuss with their loved one.
Documents to consider for end of life planning include:

Advance directive (medical decision-making before you die)

Power of attorney (financial management while you are alive)

Disposition of remains (who receives your remains after you die)
